     OrCAD PCB Designer v16.5

    I'm trying to create a padstack for the middle pad of this part:

    When using the "Multiple Drill" option in the Padstack Designer, it doesn't look like those drills are plated, nor have any option to have pads on the top and bottom layers of these drills.  Ideally, I'd like these to be vias of 0.4mm drill and, let's say 0.6mm pad and electrically connected to the 0.6x1.9mm main pad. I know I could use the "Drill/Slot hole" option to make 1 hole wherever I want with plating.  I could even achieve the top rectangle pad, with a multi layer stackup, the 1 drill in the middle with a circular pad defined on the bottom, but then the other 2 drills elude me.

    My fallback is just to do the main pad as described above with the 1 drill and then add the remaining 2 vias in the design, but that seems like a recipe for problems if it's not part of the part's footprint.

    Any advice and/or does anyone have a padstack like this already that they would be willing to share?

    Just add them as vias in the symbol. This kind of requirement is very common with QFN pachages and is easily handled with vias which will pick up the pad net in the design. Just ensure that your centre pad shape touches the vias.
